Understanding Alopecia Areata: Why Hair Loss Might Be an Immune Response

Introduction

Alopecia Areata is a chronic immune-mediated type of autoimmune disease involving the hair follicle, nail, and sometimes the retinal pigment epithelium This condition attacks the person’s hair follicles in the anagen and causes hair loss but does not permanently damage the follicles. Alopecia areata is caused by an autoimmune disorder of the normal hair cycle that causes loss of immune privilege in the hair follicles.

For males and females, the average age of onset is 32 and 36 years, respectively. 

Vitiligo, lupus erythematosus, psoriasis, atopic dermatitis, thyroid disease, and allergic rhinitis are among the dermatological and systemic disorders that have been linked to Alopecia Areata. 

Hair Cycle & Science Behind Alopecia Areata  

Anagen, catagen, and telogen are the three stages of the hair cycle. While the proximal end of the hair shaft is keratinised during the catagen phase, the hair is actively developing during the anagen phase. The remaining proximal end then experiences apoptosis, and the hair follicle falls out. The time between the old follicle's regression and the start of the subsequent anagen is known as the telogen. 

During the anagen phase, hair grows in six stages. The completely developed anagen follicle is stage VI. Alopecia Areata, on the other hand, causes abrupt hair loss and a lack of hair regeneration because the hair follicles are stalled in stage III or IV and prematurely return to the catagen or telogen phase. 

Types Of Alopecia Areata  

Alopecia Areata can occur in different types or forms and they cause different amounts of hair loss. It usually presents as a patchy, isolated loss of hair.

There are three 
main types of Alopecia Areata [2]:

1. Alopecia Areata (Patchy): This patchy hair loss is characterized by one or more round or oval patches on the scalp or other hair-bearing areas of the body. The condition may progress to alopecia totalis (hair loss over the entire scalp) or alopecia universalis (hair loss over the whole body). 

2. Alopecia Areata Totalis: Alopecia totalis causes hair loss over the entire scalp. 

3. Alopecia Areata Universalis: It is a condition that causes hair loss all over the body including eyebrows and eyelashes. 

Other Types of Alopecia Areata:

 1. Diffuse Alopecia
Areata:
 Diffuse alopecia areata is the sudden, unexpected thinning of hair over the entire head. It is difficult to diagnose because it looks like other types of hair loss, such as telogen effluvium or male or female pattern hair loss. 

 2. Ophiasis Alopecia: Hair loss is in the form of a band around the sides and back of the head. 

Pathophysiology  

The onset of Alopecia Areata is strongly associated with the loss of immune privilege in hair follicles, resulting in immune dysregulation. Current evidence suggests that the cause of the condition is autoimmune, with a substantial genetic contribution, further modified by unknown environmental factors. Reported triggers include emotional or physical stress, vaccinations, viral infections and medications. These triggers initiate the loss of immune privilege through inhibition of the synthesis of 2 anti-inflammatory cytokines, transforming growth factor-β (TGF-β) and α-melanocyte–stimulating hormone (α-MSH). They also induce expression of the major histocompatibility complex class I (MHC-I) polypeptide-related sequence A (MICA) on hair follicles. Then natural killer (NK) cells are activated and subsequently secrete interferon-γ (IFN-γ) and interleukin-15 (IL-15). 

IFN-γ upregulates MHC-I protein expression on the hair follicle cells and exposes antigens that were previously hidden to T cells. IL-15 inhibits T-regulatory-cells, IFN-γ and IL-15 activate target immune cells through the JAK signal transducer and activator of the transcription (JAK-STAT) signalling pathway. This causes inflammatory cells to attack the matrix epithelium of the hair follicle in early cortical differentiation or anagen hair follicles and causes a premature transition into the catagen or telogen phase. 

Causes Of Alopecia Areata    

  • Alopecia Areata is an autoimmune disease, meaning your immune system attacks your body. Your immune system attacks your hair follicles because it believes they are foreign invaders like bacteria, viruses, parasites or fungi that cause infection, illness and disease.

  • Dermoscopy is a useful supplementary diagnostic technique for diagnosis confirmation.  

  • Broken hair, yellow dots, black dots, exclamation point hairs, and short vellus hairs are early indicators of regrowth. The lack of hair follicles indicates cicatricial or scarring Alopecia. You can do a hair pull test to verify active hair loss.  

  • Results from examining the nails may also confirm the diagnosis.  

  • A skin biopsy at the edge of an active area of hair loss may be helpful if the diagnosis is unclear.   

Management and Treatment of Alopecia Areata 

Spontaneous regrowth of hair occurs within a year in about half of patients with Alopecia Areata without treatment. First-line therapies for those seeking intervention include intralesional and topical corticosteroids. Topical betamethasone is often used, especially in children, but should be discontinued if there is no improvement after 3 months. For more severe cases, topical immunotherapy or oral JAK inhibitors may be more effective.
